Forms & Schedules

New Mexico certified operators and samplers who are associated with a New Mexico public water system may apply to be able to log in to the online Drinking Water Sample Collection Application, which provides guidance, schedules and forms for public water system sampling. Sampling schedules can also be found on the Drinking Water Watch.

Public water systems should only register the operator that is responsible for collecting and tracking the water quality samples. Operators and samplers must have current certification, plus be registered with the appropriate public water system in the New Mexico Utility Operator Certification Program Database.  Operators’ and samplers’ certification and status will be verified prior to issuance of log-in credentials.

New Required Chain-of-Custody Sample Forms

Beginning January 1, 2019, Total Coliform (TC) laboratories must begin utilizing a new mandatory Chain-of-Custody (COC) for submission of all microbiological samples for public water systems in New Mexico. This new COC is designed to meet the minimum sampling requirements for the Ground Water Rule (GWR), Revised Total Coliform Rule (RTCR) and the LT2 rules.
