Electronic Reporting Form for Employers: Injury Tracking Application or ITA

OSHA has provided employers with a website that offers three methods for reporting an incident. The first being a webform where a user can manually enter data, second would be the user can upload CSV files for single or multiple establishments, and lastly the users using automated recordkeeping systems will have the ability to transfer data electronically via Application Programming Interface of API. The ITA launch page is where a user account can be created and is where OSHA Form 300A information can be submitted.

Please follow the directions below to report to OSHA work-related fatalities, hospitalizations, amputation or loss of an eye.

OSHA announced updates to its recordkeeping rule on Sept. 11, 2014.

OSHA’s updated recordkeeping rule expands the list of severe injuries and illnesses that employers must report to OSHA. As of Jan. 1, 2016, all employers under NM-OSHA jurisdiction must report:

  1. All work-related fatalities within 8 hours.
  2. All work-related inpatient hospitalizations, amputations and losses of an eye within 24 hours.

Among the changes, employers will have the option to file online reports of fatalities and severe injuries and illnesses via a form on this page. The electronic form can be accessed using the ITA launch page.

Other methods employers can report accidents to NM-OSHA:
